THIS WORKSHOP WILL ENABLE WORKERS TO:
· Describe the importance & benefits of Advance Care Planning (ACP)
· Explain the context of ACP
· Know the value and limits of care worker involvement in ACP
· Develop skills in starting ACP conversations
· Recognise opportunities for ACP referrals and how to prepare clients
